This forms contain example orbit calculations and blank "worksheets" for each instrument, that can be used to help lay out the exposures and overheads needed to calculate the number of orbits required. Detailed instructions for how to make the calculations are provided in section 18 of the Call for Proposals.
Note that these worksheets are not for submission with the Phase I proposal, but are strictly for your convenience for calculating the number of orbits.
